What is Flooding?
Flutide is an inhalative corticoid (ICS) of GlaxoSmithKline, which contains the active ingredient Fluticasonpropionate. It is used for long-term treatment of bronchiale asthma and is one of the most effective inhalative corticoids.
Fluticason has a higher active strength than Budesonid or Beclometason and is therefore used in lower doses. It acts almost exclusively locally in the airways and has hardly any systemic side effects when used correctly.
Flutide is available as a dosing aerosol and as a discus (powder inhaler).
Active Ingredients & Mechanism of Action
Active ingredient: Fluticasonpropionat
Fluticason acts as anti-inflammatory in the airways:
Active mechanism: 1. Fluticason binds to glucocorticoid receptors in the cells Two. The complex migrates into the cell nucleus 3. Anti-inflammatory genes are activated 4. Inflammatory genes are inhibited
Effects:
- Reduces inflammation cells in the airways
- Reduces mucous membrane swelling
- Reduces bronchial hyperreagiability
- Prevents remodeling of airways
Pharmacokinetics:
- High lipophilia – good lung permeability
- Low oral bioavailability (<1%)
- Almost complete first pass metabolism
- Therefore little systemic effects
Who is it suitable for?
Floodide is suitable for:
- All grades of Asthma
- As a basic therapy (controller)
- Children from 4 years (Diskus from 4, spray from 1 year)
- Patients who are not sufficiently controlled with low dose ICS
Advantages:
- High effective strength – lower doses required
- Low systemic effects
- Various dosage forms
Restrictions:
- No acute effect on breathing
- Full effect only after days to weeks
Available Dosages
Flutide Diskus:
- 50 μg, 100 μg, 250 μg, 500 μg per single dose
Flutide dosing aerosol:
- 50 μg, 125 μg, 250 μg per stroke
Dosing with asthma (adults):
- Easy: 100-250 μg 2x daily
- Medium: 250-500 μg 2x daily
- Heavy: 500-1000 μg 2x daily
For children: Lower doses depending on age and severity.
equivalence doses: Fluticason is about twice as potent as Budesonid or Beclometason.
How to Take
Flutide Diskus: 1. Open the disk (slide to the thumb support, to the outside) Two. Push back lever until it clicks (dose ready) 3. Breathe deep (not in the discus) 4. Close the mouthpiece Five. Breathe uniformly and deeply 6. Keep breath 10 seconds 7. Close the disk, rinse your mouth
Dosieraerosol:
- With Spacer recommended for better active substance deposition
- Coordination between spraying and inhaling
- rinse mouth after inhalation
Important:
- Apply regularly, even without complaints
- rinse mouth after each inhalation (mouth soor prevention)
- Do not drop abruptly
Contraindications
Do not apply at:
- Hypersensitivity to Fluticason
- Acute asthma attack (no immediate effect)
Preview at:
- Active lung tuberculosis
- Mushroom infections of the respiratory tract
- Heavy liver failure (delayed degradation)
- When using CYP3A4 inhibitors (e.g. Ritonavir)
pregnant: Can be used with appropriate indication. Asthma control is more important than theoretical risks.
Style time: Application possible. Low amounts in breast milk.
Possible Side Effects
Acid (1-10%):
- Oral Candidose
- Heiserkeit
- neck irritation
- Coughs
Occasionally:
- Headaches
- Taste disorders
- Skin rash
Selten (at high doses/long-term therapy):
- Adrenal suppression
- slowing growth in children
- Bone density reduction
- Cataract, glaucoma
- Changes in behaviour (children)
Prevention:
- rinse mouth after inhalation
- Use the lowest possible effective dose
- Regular checks on children
Interactions
Reactions:
- Ritonavir and other strong CYP3A4 inhibitors – strongly increased Fluticason exposure, Cushing syndrome possible!
- Ketoconazole, Itraconazole – increased Fluticason mirror
- Cobicistat – similar interaction to Ritonavir
Important: In HIV therapy with Ritonavir or Cobicistat: Avoid floodicason-containing inhalers as much as possible, alternatively use beclometason.
