What is alvesco?
Alvesco is an inhalative corticoid (ICS) which contains the active ingredient Ciclesonid. It is a prodrug that is only converted into active active ingredient in the lungs – as a result, fewer local side effects such as the oral soor occur.
Ciclesonid has a high lipophilia and a long working life, so that Alvesco has to be inhaled only once a day. It is particularly well tolerated and suitable for patients suffering from heel or mouth soor.
Alvesco is available as a dosing aerosol in various strengths.
Active Ingredients & Mechanism of Action
Active ingredient: Ciclesonid
Ciclesonid is a prodrug with local activation:
Activation: 1. Ciclesonid is hardly active Two. In the lung it is converted by esterases to the active desisobutyryl-Ciclesonid (des-CIC) 3. Des-CIC has a 100-fold higher affinity for the glucocorticoid receptor
Prodrug benefits:
- In the mouth hardly activation → less mouth soor
- Very high lung speed
- Low system availability
- single-day dosage possible
Inflammatory action:
- Reduces inflammation cells and mediators
- Reduces mucous membrane swelling
- Lower bronchial hyperreagiability
Who is it suitable for?
Alvesco is suitable for:
- Asthma bronchiale in adults and adolescents from 12 years old
- Patients with local side effects among other ICS
- If daily dosage is desired
Advantages:
- Only once a day
- Less mouth soor and hoarseness
- Good compatibility
- High lipophilia
Restrictions:
- Not allowed for children under 12 years
- Not for acute treatment
Available Dosages
Alvesco dosing aerosol:
- 80 μg per stroke
- 160 μg per stroke
Dosing at Asthma:
- Light to medium weight: 80-160 μg once a day
- Medium to heavy: 320 μg once a day (2 strokes of 160 μg)
- Maximum: 640 μg per day (same required)
Time: Once a day, preferably in the evening.
How to Take
Dosieraerosol: 1. Remove protective cap Two. Spray well shake 3. Deep exhalation 4. Closing mouthpiece with lips Five. Breathe slowly and deeply, spray 6. Keep breath 10 seconds 7. Breathe slowly
With Spacer:
- Spray spray in Spacer
- Normally breathe 5-6 times
- Improves active substance deposition
Important:
- Apply regularly, even without complaints
- rinse mouth after inhalation (even if risk is lower)
- Note equivalence doses when changing ICS
Contraindications
Do not apply at:
- hypersensitivity to ciclesonide
- Children under 12 years
Preview at:
- Lung tuberculosis
- respiratory infections
- Heavy liver insufficiency (prodrug activation could be altered)
pregnant: Limited data. Only with clear indication.
Style time: Unknown whether it goes into breast milk.
Possible Side Effects
Acid (1-10%):
- Headaches
- Nasopharyngitis
Occasionally:
- Dysphonie (heiserness) – but less often than with other ICS
- Mundsoor – rarer than other ICS
- Coughs
- nausea
Selten:
- Paradox bronchospasmus
- Allergic reactions
Systemal effects: With high doses and long-term therapy possible, but less than with other ICS.
Interactions
Reactions:
- CYP3A4 inhibitor (Ketoconazole, Ritonavir) – increased ciclesonide exposure possible
- Less than in Fluticason, since Ciclesonide is metabolized less via CYP3A4
No relevant interaction with:
- Most popular medicines
- Hormonal contraception
